Photography Equipment Use and Checkout Permission Slip

 

The photography program at Streamwood High School is extremely fortunate to be able to offer our students the opportunity to use and checkout quality 35mm adjustable cameras, digital cameras, and other pieces of photographic equipment. Maintaining a collection of cameras and equipment is an expensive endeavor, therefore it is extremely important that students take great care in handling the tools provided.

 

  1. Upon reading and signing this contract, photography students are granted the privilege to use photographic equipment owned by the photography program at Streamwood High School.
  2. Students in Photography 1-2, 3-4, and 5-6, who have read and signed this contract and remains in good academic standing in the photography program, are allowed to check out photographic equipment owned by the photography program at Streamwood High School.
  3. Any student who does not abide by the rules of the photography classroom, or continually abuses equipment will loose access to the school’s cameras and other photographic equipment. In order to continue within the photography program, the student will have to find another source for camera use.
  4. If a camera or other equipment is lost, stolen, or damaged the student who checked those items out will bear the total cost of replacement. Cost of replacement will be based upon the current market value of the piece of equipment checked out by the student. In the case of equipment damage, the student may choose to provide full payment for the cost of replacement or can take the piece of equipment to be repaired by a professional repair company approved by Ms. Cress. If the later option is chosen, the student has full responsibility in making repair arrangements. Final approval of the equipment will be at the discretion of Ms. Cress.
  5. All cameras and photographic equipment are to be used only by photography students.
  6. Cameras and photographic equipment should not be taken on long trips in which they can easily be left behind.
  7. Cameras and other photographic equipment may be checked out at the end of the school day, on a first come, first serve basis after the 2:55 bell (Two cameras will be reserved for seniors who leave after sixth hour on a first come, first serve basis.)
  8. Cameras and other photographic equipment must be returned the following morning before first period (7:30 – 7:40 am). There will be no exceptions. Cameras must be returned to the teacher directly. Cameras cannot be left on the teacher’s desk or in the classroom. A student who is ill must still return the equipment checked out, whether through a friend or relative, and that student is still responsible for damages. The main office will accept a camera from a parent.)
  9. Cameras and other photographic equipment may be checked out over the weekend. The checkout period for weekend loans is Friday after the last bell Until Monday before school.
  10. Any student who does not return photographic equipment on time will be assigned one darkroom cleaning period before school, after school or during a free period during the day. Students will receive additional darkroom cleaning periods for each day equipment is late.
  11. Cameras and other photographic equipment cannot be checked out over Winter Break or Spring Break.
  12. Cameras and other photographic equipment should not be taken to a camera repair service without the knowledge of the supervising teacher (Ms. Cress). If cameras or other photographic equipment is tampered by a third party without the knowledge of Ms. Cress, other than the student who checked the items out, the student will bear the total cost of replacement.
